In short: Free NHS prescriptions in England are available to under-16s, over-60s, pregnant women and new mothers, people with certain medical exemptions, NHS inpatients, and those on qualifying benefits including Pension Credit or with a valid HC2 certificate. Always check eligibility before ticking the exemption box.

Age-based exemptions cover children under 16 (or under 19 in full-time education), people aged 60 and over, and women who are pregnant or have had a baby in the previous 12 months with a valid maternity exemption certificate. These apply automatically when you show proof at the pharmacy.
Medical exemption certificates cover specific long-term conditions including diabetes treated with medication, epilepsy, hypothyroidism, cancer, and several others listed on form FP92A. Apply through your GP; the certificate lasts five years. Separate arrangements apply for war pensioners and Ministry of Defence exemption holders.
Income-based help includes Pension Credit (Guarantee Credit, alone or with Savings Credit), Universal Credit where earnings are below the threshold set by the NHS Business Services Authority, and holders of a valid HC2 certificate issued after a low-income assessment (form HC1). HC3 certificates give partial help with health costs rather than full exemption.
Prescriptions are also free in Scotland, Wales and Northern Ireland for all residents. In England, claiming a free prescription you are not entitled to triggers a penalty charge — currently up to five times the prescription cost plus the original charge. Use the NHSBSA online checker before ticking an exemption box, and keep certificates up to date.
