Subject Access Request (UK GDPR)

Use this template to request a copy of the personal data an organisation holds about you, under Article 15 of the UK GDPR. The organisation must respond within one month and cannot normally charge a fee.

- Send the request to the organisation's data protection officer or privacy team. Use email where possible so the one-month clock starts on receipt.
- You are entitled to a copy of the personal data, plus information about why it is held, who it is shared with, where it came from and how long it will be kept.
- The organisation can extend the response window by up to two further months for complex requests, but must tell you within the first month.
- If the response is inadequate or absent, you can complain to the Information Commissioner's Office (ICO) at ico.org.uk.
